DELIVERABLE:
My Health and Home Connection Pilot
LDC established a multi-state pilot, My Health and Home Connection (MHHC), to address the needs of high utilizers of emergency room services while reducing homelessness. The pilot, active in California, Florida, New York, and Texas, involved partnerships with leading community-based organizations to provide intensive case management services.
MHHC care plans, developed by LDC, responded to each person’s unique circumstances, focusing on strengths-based, trauma-informed, and harm-reduction approaches. Together with Anthem and CBOs, LDC aimed the MHHC program at securing stable housing for participants.
